How To Submit Forms
For the 2020-21 Academic Year, students have two options to submit their Financial Aid Forms
- Secure Upload **Preferred Method**
- You may upload your forms using the secure upload tool on our webpage.
- Go to Form Submission and follow the instructions to upload your financial aid forms.
- Submit at Financial Aid Office
- We have modified office hours during the pandemic emergency. We are open to the public from Monday through Thursday from 8 am to 3 pm and on Fridays from 8 am to 12 noon.

| FAO Information Sheet | Collects student demographic information and provides for FERPA exceptions. Required to be submitted once every Academc Year. |
| DEPENDENT Verification Worksheet (Group 1) | For Dependent students selected for verification under verification Group 1 |

| Dependent Non-Tax Filer Form | For dependent students whose parents and/or themselves do not intend or are not required to file a tax return. |

| Independent Non-Tax Filer Form | For independent students who do not intend or are not required to file a tax return. |
| Instructions for Requesting Federal Student Loans | Detailed instructions for completing your Master Promissory Note and Entrance Counseling required to request federal student loans. |
| Federal Loans Change Form | Form to request change in Direct Loan award amounts. |
| Request to Review Financial Need Status | Form to submit to request revision of financial need status due to changes in income levels. |
| Satisfactory Academic Progress Appeals Form | Form required to submit to request reinstatment of financial aid after failing to meet the reguirement of the Satisfactory Academic Progress Policy. |
